Please read the below carefully. This is the final step in acknowledging that you are happy with the work delivered by DVM Development. Once this form has been signed you will have one week of free changes, there after changes will be charged hourly.

Accepting this form means that you acknowledge that any modification or new work to the approved website will incur additional charges and will need to be scoped unless stated otherwise.

This form confirms that the project is now complete and DVM Development has been fulfilled the brief to the clients satisfaction.
